Applications are due November 15th.  Completed applications should be mailed to:

Be sure to carefully follow all instructions included with the applications.  Also included with the downloadable applications below are program details, cost information, and expectations.  If you have questions, please contact the Wisconsin 4-H International Programs office at 608-262-2491.

Medical forms for both delegates and adult advisors ARE REQUIRED for International Programs. Please make an appointment with your doctor to fill out the form ASAP, as these forms are due NO LATER THAN February 25th to the WI 4-H International Programs Office. Please note that applicants applying for travel to Japan fill out a different medical form than those applying for other countries (see below).

Delegates (Youth)

The completed application includes:

  • Completed and signed application form
  • Photo (email to wi4hinternational@extension.wisc.edu preferred)
  • Comprehensive Release Form (signed)
  • Letter of Reference from your school (teacher, principal, etc.)
  • Letter of Reference from your community (4-H leader, civic leader, employer, etc.)
    • We strongly recommend using a 4-H leader or your county educator for a reference if possible. Regardless, we ask that delegates reach out to their county 4-H office during the application process so the county educator can provide appropriate support and resources.
  • Delegate Interview – not sure who can interview?! Click HERE for a list of our approved interviewers and where they’re located!
  • Partial Program Fee of $250 (payable to University of Wisconsin Extension)
  • Program deposit (amount varies by program; payable to States 4-H International Exchange Programs)

Items due after Nov. 15th:

  • February 1: Second payment due*: $2,000 for Japan 8-week, $1,500 for Japan
    4-week, South Korea, Taiwan, Romania & Norway, and $1,000 for Costa Rica
  • March 1: Medical Form and passport copy due
  • April 15: Remaining payment due

  • Mid-Winter: Following a successful interview, States’ 4-H notifies applicants of their acceptance
  • February 1: Balance of Program Fee due (payment plans are possible)
  • March 1: Medical Form and Passport Copy due
